When to refer to Visionwest Home Healthcare
There is support available to enable you to stay in your own home if you are struggling with day-to-day tasks due to an ongoing illness, age-related health challenges or you are living with a disability.

How to refer to Visionwest Home Healthcare
ACC Nursing and Wound Care
You can refer yourself directly to Visionwest.
If you need care for an approved ACC-covered accident or injury, and you find visiting your GP difficult, you can refer directly to Visionwest – fill in the form below.
Other Home Healthcare Services
A NASC assessment is required first.
For In-Home Aged Care, Disability Suppoprt, Individualised Funding, Respite Care, Chronic Health Support, Personal Care and Home Help – you must first be assessed by Needs Assessment and Service Coordination (NASC).
If you are eligible for funded support, ask to be referred to Visionwest.

- You have funding approved by NASC for home healthcare support services.
