We understand how important it is to stay in control of your recovery and rebuild your confidence. Every service we offer is tailored to your unique needs and goals, helping you regain your independence and get back to doing the things you love – at home and in your community.
What type of in-home support is available?
- Personal Care: Showering, dressing, and grooming, medication, assistance with eating anddrinking.
- Household Management: Meal preparation, cleaning, laundry, and grocery shopping.
- Staying Active: Exercise, mobility assistance, rehabilitation and transport.

Can I receive ACC funded support and also access Private Care services?
Yes. Sometimes support needs aren’t fully covered by ACC funding. In these cases, Visionwest Home Healthcare can provide additional Private Home Care services.

ACC Home and Community Support to Maximise Independence
Visionwest Home Healthcare is an approved provider of ACC Home and Community Support to Maximise Independence.
This service helps you recover from injury, regain control over daily life, and rebuild confidence at home and in your community.
We also partner with trusted Allied Health professionals and offer Nursing Services to enhance your recovery and support your long-term wellbeing. Once ACC refers you to us, one of our Care Managers will meet with you and your whānau to create an Individual Support Plan (ISP) tailored to your recovery goals.

What you can expect
When you’re referred to us, we’ll work with you to create a personalised plan that fits your goals, lifestyle, and recovery needs. This plan is called an Individual Support Plan (ISP). It brings together everything you need for your rehabilitation, from daily support to nursing and allied health services, in one clear plan.
You’ll have input every step of the way, and we’ll also collaborate with your whānau, Support Workers, and any health professionals involved to ensure everyone’s on the same page.
Your plan will be reviewed regularly to make sure it continues to meet your needs. Because we want you to feel fully supported and in control of your care.
Allied Health Services
Our allied health partners will provide rehabilitation and recovery support to help you regain strength, mobility and independence after an injury. We’ll also review your progress regularly, adjusting your plan to make sure you’re always moving forward at a pace that feels right for you. Our in-home allied health services include:
- Physiotherapy: Improving mobility, strength, and balance after injury or surgery.
- Occupational Therapy: Supporting daily activities, recommending equipment, and adapting your home for greater safety and independence.
Working closely with our Visionwest Support Workers, they ensure your Individualised Support Plan (ISP) is followed and that every aspect of your wellbeing is supported – helping you regain confidence, independence, and quality of life.
Nursing Services
If you need nursing support as part of your recovery, our experienced Registered Nurses will work with you to make sure your care is safe, coordinated, and effective. We’ll start with a nursing assessment to understand your needs, then create a care plan that fits seamlessly into your Individual Support Plan (ISP).
Our nurses can help with wound care, medication management, and health monitoring, as well as provide guidance for Support Workers and whānau to assist in your rehabilitation. We’ll also share strategies to prevent future injuries , such as fall prevention or pressure care, so you can stay well and independent at home.
Everything we do is designed to support your recovery journey with compassion and professionalism.
How to apply for ACC Funded Care
You can apply to ACC for home support once your injury claim has been accepted.
1. Get a referral.
Ask your doctor to send ACC a letter recommending home support. ACC will contact you once this is received.
2. Apply to ACC.
If your recovery is expected to be short, your recovery team may approve support straight away. For longer recoveries, ACC will arrange an assessment to determine what help you need.
3. Assessment.
An independent assessor will contact you within two working days to book a home visit (or at your temporary accommodation). The assessment will be completed within ten working days.
4. ACC decision.
ACC will contact you within seven working days of the assessment to discuss recommended support, approval details, and next steps.
5. Choosing your support.
You can receive help from:
a contracted caregiver through an agency
someone you know (friend or whānau)
or a combination of both.
