According to Health New Zealand, Te Whatu Ora, one in four New Zealanders live with multiple chronic long-term conditions. These illnesses often occur across several generations within the same whānau, with Māori and Pacific peoples facing a higher risk.
The level of care required varies depending on the condition, and its severity. Each case is unique, and support plans are tailored to individual needs.

How to access Chronic Health Support
If you are living with a chronic health issue, you may be eligible for in-home support services, such as personal care, home help, and medication oversight, all delivered by our qualified Visionwest Support Workers.
These services are available free of charge to those who qualify for government-funded support. Access to these supports can be arranged through Te Whatu Ora: Health NZ, MSD Disability Support Services, and ACC.
For those not eligible for government funding but who wish to receive support, we also offer privately funded services.
